New Patient Information

Before your first visit with Dr. Wolfe, please download the Privacy Notice and the Intake Packet by clicking the buttons at the bottom of this page. Carefully read the Privacy Notice as this outlines your rights under the Health Insurance Portability and Accountability Act. Then print and complete the Intake Packet in its entirety and bring it to your first appointment. Please DO NOT staple or hole-punch the Intake Packet.

If you bring a completed Intake Packet to your first appointment, you need arrive only 10 minutes before your appointment time.

If you are unable to bring a completed Intake Packet, you must arrive 20-30 minutes before your appointment time. There will be a copy of the Intake Form and Privacy Notice in the waiting room. When you arrive, please complete the Intake Packet.

Payment in full is due at time of service and must be in the form of either check or cash. Credit and debit cards are not accepted.

If your visit will be covered by insurance, you must bring your insurance card with you. Most major Insurance Plans are accepted.
