New Patient Information

If you are scheduled for a first visit with Dr. Wolfe, please read the Notice of Privacy Practices and download the Intake Questionnaire by clicking the buttons at the bottom of this page. Carefully read the Notice of Privacy Practices as this outlines your rights under the Health Insurance Portability and Accountability Act. Then print and complete the Intake Questionnaire in its entirety and bring it to your first appointment. Please do not punch holes in or staple the Intake Questionnaire.

If you bring a completed Intake Questionnaire to your first appointment, you need arrive only 10 minutes before your appointment time. Please DO NOT staple or hole-punch the Questionnaire.

If you are unable to bring a completed Intake Questionnaire, you must arrive 20-30 minutes before your appointment time. There will be a copy of the Intake Questionnaire and the Notice of Privacy Practices in the waiting room. When you arrive, please complete the Questionnaire.

Payment in full is due at time of service and must be in the form of either check or cash. Credit and debit cards are not accepted.

If your visit will be covered by insurance, you must bring your insurance card with you. Most major Insurance Plans are accepted for individual psychotherapy.
